Making a list of items report generated by checking boxes

I've you've ever tried inventorying a set of trading cards or other numbered items, it's a pain and time consuming. Imagine having a group of check boxes that you can just check off what you have or what you need.

After you've checked off the relevant numbers, a list will be returned based on the user's display preferences.

I'm hoping that there's someone out there who can make the inituitive leap to get a working version online and/or for Mac OS X.
